Evaluation of true maximal oxygen uptake based on a novel set of standardized criteria

Abstract

Background. Criteria are used to identify whether a subject has elicited a VO2max. Objective. Evaluate the validity of traditional VO2max criteria and propose a novel set of criteria. Research design and methods. Twenty athletes completed a VO2max test consisting of an incremental phase and a subsequent supramaximal phase to exhaustion (verification phase). Traditional and novel criteria were evaluated. Novel criteria were: 1) VO2 plateau defined as a difference between modelled and actual VO2max >50% of the regression slope of the individual VO2-workrate relationship; 2) same as criterion 1, but for maximal verification VO2; 3) difference ≤4 beats∙min-1 between maximal heart rate values in the two phases. Results. Satisfying the traditional VO2 plateau criterion was largely an artefact of the between-subject variation in the VO2-workrate relationship. Secondary criteria, supposedly an indicator of maximal effort, were often satisfied long before volitional exhaustion, even as low an intensity as 61% VO2max. No significant mean differences were observed between the incremental and verification phases for VO2max (t = 0.4; p = 0.7) or heart rate (t = 0.8; p = 0.5). The novel VO2 plateau criterion, verification criterion and maximal heart rate verification criterion were satisfied by 17, 18 and 18 subjects, respectively. The small individual absolute differences in between incremental and verification phases, observed in most subjects, provided additional confidence that VO2max was elicited. Conclusion. Current VO2max criteria were not valid and novel criteria should be further explored.
